New Patient Forms
Now that you have made your first new patient appointment with Cosentino Dental Care there are a few things you need to know: Whether you are searching for a new dentist or a second opinion, we are happy to discuss all your dental care needs with you. Upon arrival, we will ask you to complete our new patient forms. Please arrive up to 15 minutes before your scheduled appointment to ensure you have time to do so. We need to know about your dental and overall physical health history so that we can offer you the best care and treatment plan.
New Patient Appointment
During your appointment, you will discuss your oral health goals with Dr. Cosentino. It is important to ask any questions and mention any concerns you have. But don’t worry, we are always here to answer any questions you may have.
Dental Coverage
We accept most insurance plans as well as the Canadian Dental Care Plan so please let us know if you have dental coverage. We’ll also help you make the most of your plan and answer any coverage questions to the best of our abilities during your visit.
